PROMIS-57 v2.1

The PROMIS-57 survey measures pain interference and pain intensity by administering a fixed number of items from 7 PROMIS domains: depression, anxiety, physical function, pain interference, fatigue, sleep disturbance, and ability to participate in social roles and activities. There are 8 questions per domain, plus an additional pain intensity 0-10 numeric rating scale. The profiles are universal rather than disease-specific. They assess all domains over the past 7 days, except for physical function, which has no time frame specified. The PROMIS 57 v2.1 has been validated for use with kidney transplant recipients.

Kidney Disease Quality of Life (KDQOL-SF) v1.3

The KDQOL is a kidney disease-specific survey assessing health-related quality of life. It contains 36 questions centered around symptoms/problems, effects of kidney disease, burden of kidney disease, work status, cognitive function, quality of social interaction, sexual function, sleep, social support, staff encouragement, and patient satisfaction.

Perceived Stress Scale

This survey measures the degree to which an individual appraises their life circumstances during the previous month as stressful. It consists of 10 items using a 5-point Likert scale ranging from 0 (never) to 4 (very often).

VioScreen is an on-line dietary questionnaire that gathers information about your diet. Easy to follow prompts will guide you through the process of collecting information about the foods you eat on a daily, weekly and monthly basis.
